2019 Biennial Member Survey

It is time again for the Biennial Member Survey.  The purpose of this survey is to gather your thoughts, opinions, and ideas on our trainings and activities. We want to make sure we are serving your needs as diabetes health professionals and educators.
The survey should take approximately 10-15 minutes to complete.  All responses will be kept confidential.   

If you would like to be entered into the drawing for a $50 gift card to WalMart, please make sure you enter your name and e-mail at the end of the survey.
All surveys must be returned by September 30 to be eligible for the drawing.
